San Gabriel Valley Best Babies Collaborative
The San Gabriel Valley Best Babies Collaborative (SGVBBC) is a project of six community agencies working together to improve birth outcomes of families in the East San Gabriel Valley. The SGVBBC’s mission is comprised of culturally diverse health and community-based organization committed to serving and empowering high risk women, of reproductive age, in the San Gabriel Valley. SGVBBC provides intensive case management to 60 high-risk women in Baldwin Park, El Monte, La Puente, and Pomona (zip codes 91732, 91706, 91744, and 91766).
Citrus Valley Health Partners (CVHP) is the lead agency of the six partners that make up the San Gabriel Valley Best Babies Collaborative (SGVBBC) and is a family of hospitals and a hospice that serves nearly one million residents in the East San Gabriel Valley. Our four locations offer comprehensive health care services, guided by our mission, to keep people well in body, mind and spirit. CVHP is comprised of Citrus Valley Medical Center - Inter-Community Campus in Covina, Citrus Valley Medical Center - Queen of the Valley Campus in West Covina, Foothill Presbyterian Hospital in Glendora and Citrus Valley Hospice in West Covina.Other partners includes East Valley Community Health Center; Foothill Family Service; SPIRITT Family Services; Catholic Charities and Public Health Foundation Enterprise/WIC.
The San Gabriel Valley Best Babies Collaborative is comprised of culturally diverse health and community-based organizations committed to serving and empowering high-risk women, of reproductive age, in the San Gabriel Valley. Our purpose is to optimize birth and maternal outcomes through comprehensive and integrated health and wellness services.
The San Gabriel Valley Best Babies Collaborative strives to be the region’s premier source of knowledge, advocacy, and service provision to promote and foster a community of healthy women, healthy births, and healthy families.
